2.  Pick a day Monday, Tuesday, Wednesday or Friday and Time between 9am and 12 and 3pm and 6

3.  Type of appointment either New Patient or Returning Patient

4.   Fill  in your information

5.  New to the office be sure to print out the new patient information sheet - fill that out and bring it with you to your first appointment
